• My Number card (if the information is updated)
• Other official documents proving name change

For address changes, please submit one of the following documents (issued within the last 6 months with your new address):

• Resident registration
• Health insurance card
• Utility bill/Invoice
• Tax certificate
• Phone bill/Invoice
• Insurance statement/Invoice
• Credit card statement
• Bank balance certificate
• Other proof of address
